The overwhelming majority of reported issues with subcutaneous peptide use are local injection site reactions rather than systemic effects. A 2023 systematic review and meta-analysis of randomised-controlled trials of subcutaneous biologic injections — covering 158 studies — found redness (erythema) accounted for 42.8% of all reported reactions, followed by unspecified reactions (23.3%), pain (12.4%), and itching (5.7%).
Overall prevalence of any injection site reaction ranged from under 1% to around 15% depending on the specific compound, and reactions typically appear within 30 minutes to a few hours of injection, resolving within one to three days on their own. Reactions also tend to become smaller and less frequent after four to eight weeks of regular injections, as local tissue adapts.
Importantly, the same review found that injection site reactions rarely led to patients discontinuing treatment — in the large majority of studies that tracked discontinuation, none was attributable to injection site reactions alone. Rotating injection sites and using proper technique meaningfully reduces the frequency and severity of these reactions.
